This Privacy Policy describes how information is collected and used as part of the Dynamic Home Health Care patient outreach program, operated in partnership with Keruu Inc.'s Linda Platform. The program uses automated voice calls, voicemail messages, and informational SMS messages to support post-discharge patient follow-up.

Phone number(s) you provide to Dynamic Home Health Care
Records of consent and opt-out
Call interaction data (such as responses or call outcomes)
Voicemail content and transcripts
Call transcripts and interaction metadata
SMS delivery and interaction metadata
Limited Protected Health Information (PHI) necessary to operate the program
How We Use Information
Information collected through this program is used solely to:
Deliver the automated calls and SMS messages described in the Terms & Conditions
Identify when you may benefit from additional support from the Dynamic care team
Operate, evaluate, and improve the automated communication technology
Maintain consent and opt-out records as required by law
Support compliance and system monitoring
Information is not used for marketing or sales purposes.
Sharing of Information
Information may be processed by trusted service providers — including Keruu Inc. (operator of the Linda Platform) and its sub-processor Twilio, Inc. — that support call delivery, messaging, transcription, analytics, or system operations. Keruu Inc. acts as a HIPAA Business Associate of Dynamic Home Health Care and is contractually obligated to protect Protected Health Information in accordance with HIPAA (45 C.F.R. Parts 160 and 164).
Mobile opt-in data, phone numbers, message content, and consent records are not sold and are not shared with third parties or affiliates for marketing, advertising, or promotional purposes.

Data Retention
Information is retained for the period required by applicable healthcare and telecommunications laws and Dynamic Home Health Care's records-retention policies. Opt-out records are retained indefinitely to ensure your preference is honored. Other data is securely deleted or anonymized when no longer needed.
Your Choices
You may opt out of future communications at any time by replying STOP to any SMS message, stating "Stop" during an automated call, or contacting Dynamic Home Health Care at (818) 981-0090. You may also request access to or correction of your information using the same contact method.
