Dynamic Home Health Care Patient Outreach

Program Description

Dynamic Home Health Care partners with Keruu Inc.'s Linda Platform to provide automated post-discharge follow-up communications to enrolled patients. The program uses automated voice calls, voicemail messages, and informational SMS messages to check on patient progress, share post-discharge information, and identify when additional support from the care team may be needed.

All communications are informational only and are not marketing communications.

Consent to Receive Communications

By initialing the "Consent to Automated Communications and AI-Assisted Follow-Up" section of the Dynamic Home Health Care Consent for Care form, you provide explicit written consent to receive automated phone calls, voicemail messages, and SMS messages related to this program.

Consent is voluntary and is not a condition of receiving home health care services.

Types of Communications

Enrolled patients may receive:

  • Automated voice check-in calls using interactive voice technology

  • Automated voicemail messages if a call is not answered

  • Informational SMS messages (such as pre-call notifications and post-call follow-ups)

Message & Call Frequency

Communications occur between 9 AM and 6 PM local time on weekdays. Frequency is limited and generally includes:

  • Up to five (5) automated call attempts per check-in program

  • A voicemail message if a call is not answered

  • Up to two (2) related SMS messages per call attempt (one pre-call, one post-call or missed-call follow-up)

Message and data rates may apply for SMS messages.

Nature of Communications

Communications reference your recent home health care experience for follow-up purposes. Communications do not provide medical advice, diagnosis, or treatment and should not be relied upon for medical decision-making. If you are experiencing a medical emergency, call 911 or seek emergency medical attention immediately.

Opt-Out & Help

You may opt out of communications at any time:

  • SMS: Reply STOP to opt out, or HELP for assistance

  • Voice: State "Stop" or "Opt-out" during the call, or follow the opt-out instructions provided

  • Direct: Call Dynamic Home Health Care at (818) 981-0090, or provide written notice

Once you opt out, no further automated communications will be sent for this program.

Call Monitoring & Voicemail

Calls and voicemail messages may be monitored, transcribed, or analyzed for quality assurance, system evaluation, and improvement of the automated technology. Limited Protected Health Information (PHI) is shared with Keruu Inc. under a HIPAA Business Associate Agreement for the sole purpose of operating this program.

Program Changes

Program details may change as part of ongoing improvement of the technology and care experience. Any material changes will apply prospectively.
